Stronghold registration forms

Prior to your first visit we recommend filling out all relevant forms for your session. This will save time at the reception and ensure you start on time.
Adult or Adult with Supervised Children – Acknowledgment of Risk
INDUCTIONS: Adult or Adult with Children on a Stronghold Induction
KIDS SESSION: Consent Form for a child participating on a Instructed Session

This form is to be filled in by a Parent or Legal Guardian of a child attending one of Stronghold’s Instructed Kids sessions or a Kids Birthday Party. The form only needs completing for the Child’s first instructed session at Stronghold.

Supervising Adult Novice: Form for Supervising an Adult Novice

This wavier is to be completed for 14-17 year olds who are experienced enough climbers to be in the centre unsupervised at Stronghold.  All unsupervised junior climbers will be assessed prior to entering the bouldering area.

UNSUPERVISED JUNIOR – Acknowledgment of Risk

This form is to be filled in by an adult participating in a pre-booked supervised personal training session at Stronghold Climbing Centre.
